PATIENT FORMS

Upon entering our Waukesha office, our receptionist or Doctor will welcome you. If you hadnt already filled out the forms located in 'New Patietn Paperwork', we will request you do so in our office. This information allows us to gain a better understanding of your health history and current condition. 

CONSULTATION

Dr. Jamie Mabeus escort you into the examination room, where you will have an opportunity to sit down and discuss your health concerns and ask any questions you may have. This is how Dr. Jamie will gain a better understanding of you, your condition, your goals and expectations. 

EXAMINATION

Upon completion of the consultation, Dr. Jamie Mabeus will perform an examination testing your reflexes and flexibility. Additional tests will include neurological, orthopedic, postural and physical tests. 

REPORT OF FINDINGS

At the termination of the examination, Dr. Jamie Mabeus will discuss her findings with you. Based on examination findings, Dr. Jamie will be able to give you treatment options and a treatment timeline, in addition to answering any additional questions you may have. 

TREATMENT

After the report of findings is completed, Dr. Jamie Mabeus will discuss treatment options with you, and explain how they will help your condition. This may include spinal adjustments, physical therapy modalities, or myofascial work. Treatment is always interactive, so you can express concerns at any time. 

WELLNESS AWAY FROM THE CLINIC

Prior to leaving your visit, Dr. Jamie Mabeus will suggest a wellness program to incorporate outside of treatment. This may include heat or ice instructions, certain activities or positions to avoid, and/or at home excercises or stretches to do.
